  • SocialHealth360 proactively identifies potential SDOH issues for individual patients based upon a library of SDOH datasets. Healthcare staff are notified and can discuss these potential issues with patients, including completing payer required Health Related Needs Surveys (HRSN).
  • Based on this and through the analysis of other data (e.g. clinical, etc.), SocialHealth360 then creates AI powered “next best actions” for the patient. These are actioned via SocialHealth360’s easy-to-use patient navigation workflow that allows the healthcare worker to assign and track actions (e.g. referrals to community based organizations), record patients notes and interactions, etc. The solution proactively prompts the healthcare worker on an ongoing patient to reach out to patients as required.
  • SocialHealth 360 maps the next best actions to Z codes – using a proprietary mapping tool – and generates a patient navigation plan that can be uploaded into patient records, EMR’s, and care systems so that all members of the patient care team are aware of potential issues that may impact treatment and outcomes.
  • Managers are equipped to better assign and manage workloads of staff to help prevent staff burnout. SocialHealth360 proactively sends “next best actions” to managers to better allocate staff based on patient loads, patient case mix, and upcoming patient visits.
  • SocialHealth360 monitors patient progress through the use of electronic patient reported outcomes (ePRO) capability built within the solution or can integrate with the client’s existing ePRO system.
  • Executives can better understand their patient population, key SDOH gaps, rising risk patients and be assured that they are meeting key performance obligations in various contracts.
  • All these capabilities are integrated into Microsoft Teams allowing for ongoing collaboration across the healthcare team.
